Referrals Process
Our referral and admissions process is personalised, ensuring we find the right support to meet the unique needs of the people we serve.
We have our in-house Multi-disciplinary therapy team including positive behaviour support specialists, occupational therapist, speech and language therapist, multimedia specialist and community psychiatric nurses.
We are highly responsive and can support a package of care within a mobilisation time of as little as 2 hours. We take risks where other providers likely wouldn’t.
We cover the full spectrum of needs from learning disability, autism and behaviours of concern through to mental health nursing through our different speciality divisions.
Referrals and Admissions
Commissioners or social workers typically make referrals on behalf of people seeking support. However, we welcome referrals from anyone interested in our services, whether for private care or through local authorities.
We are committed to understanding each person’s unique needs to match them with the right team within Unique Community Services.
An experienced team member will arrange a meeting with the person seeking support, their family members, and social care professionals. This personalised assessment will help us understand and plan the best support in line with their needs and preferences.
