24-hour Support from Our Expert Clinicians
The 24-hour provision of our care at home is made available to people with complex needs who require clinician-led care or who are living with a medical condition affecting their ability to excel in daily activities and require specific complex support. We offer continuous support from a consistent team of clinicians who understand the needs of the people we serve and are trained to provide high-quality complex care. Our 24-hour support ensures that all needs are met in two shifts in 24 hours.
Our team’s proactive approach to crisis intervention and prevention is rooted in the best training and practice that the Rapid Response Service provides.
The 24-hour support entails the following:
- Personal Care, where we make priority people’s preferences and capacity of receiving the care.
- Medication management – our clinicians have medication training
- Booking and supporting individuals to attend personalised health appointments
- Support to promote independence by teaching life skills such as cooking, cleaning, meal preparation, etc.
- Positive Behaviour Support Plans – proactive and reactive strategies
- Ensuring people have well-structured daily routines, engage in outdoor activities, run errands, and support individuals in having work experiences that align with their capabilities.
