At Unique Community Services, we break the cycle. We deliver person-centred, nurse-led, and therapy-driven support for people of all ages, shaped around each person’s unique journey. Our approach blends clinical expertise with carefully developed humanised models of care that encourage people to thrive in their own communities. Transitional Care Programmes allowing people to move from hospitals to their homes or supported living

A proactive in-house therapy team using strength-based and trauma-informed models (PROACT-SCIPr-UK®, PACE, PERMA, Talking Mats).
