What happens to people with multiple needs and chronic health conditions when limited responsive care and instability occur? The system’s fast response is a placement in another impersonal service or a delayed discharge. Should families wait again for their loved ones to receive inadequate support? Is this what people need?
Absolutely not! What people and their families need is humanised support delivered in their homes, the place where people are surrounded by the people and things they love.
At Unique Community Services, we break the cycle. We deliver person-centred, nurse-led, and therapy-driven support for people of all ages, shaped around each person’s unique journey. Our approach blends clinical expertise with carefully developed humanised models of care that encourage people to thrive in their own communities. What we stand for is to make a difference together with care professionals and with people’s first support system – families or guardians.
We provide:
Long-term and crisis care in the comfort of people’s homes
Bespoke care plans developed with and about the supported person.
24/7 nurse-led support and rapid response services within 2–4 hours.
Transitional Care Programmes allowing people to move from hospitals to their homes or supported living
A proactive in-house therapy team using strength-based and trauma-informed models (PROACT-SCIPr-UK®, PACE, PERMA, Talking Mats).
For commissioners, social workers, and health professionals,
UCS becomes your reliable, responsive, and human-first care partner. Through co-production, transparent care planning, and a flexible delivery model, we support people in finding stability and thriving within it.
We aim to achieve hospital avoidance through timely, tailored interventions and meaningful community involvement.
Redefining what complex care should feel like
- compassionate, connected and genuinely unique.

As a passionate and reliable person, I enjoy working at UCS simply because I want to make a difference and see everyone live their life to their full potential to achieve their aspirations, grow, and be the best they can be.

Dorcas-Luiza Mukwata
Working with UCS allows me to be the bridge between the clinicians and the families we support. Being at the core centre of identifying the right candidate with the correct skillset to support our families is vital to ensuring that person-centred care is given. We aim to be clinician-centric, meaning we put our clinicians first and value them. Those valued give out the best versions of themselves, and we get the best versions of our clinicians and our families. I truly admire that.

Carol Taylor
