Nurse-led care is a health and social care approach for people with complex care needs, which puts qualified nurses at the centre of delivering compassionate care. This approach is focused on meeting positive health outcomes and is led by nurses providing 24/7 support. It embodies the compassion, collaboration and expertise of our dedicated nurses that promote person-centred care.

As the term suggests, it puts me as a nurse at the centre of coordinating and liaising with other members of the MDT to ensure holistic care. This has a positive impact on the people we support as it centralises care and reduces risks of miscommunication, and clashing appointments.
