The main focus of our services is to support people with complex needs and chronic health conditions. All our services are tailored to the individual’s personal needs, always putting their aspirations first. 

The urgent and long-term complex care services we provide are followed by tailored care plans. Our clinicians create inclusive environments and strong relationships with the people we serve based on trust and respect. We help people regain a sense of meaning in their lives by encouraging them to lead more independent and fulfilled lives. Our end goal is to provide the support that will enable the individual to be as independent as possible and involved in their own community.

24-hour Support from Our Expert Clinicians

The 24-hour provision of our care at home is made available to people with complex needs who require clinician-led care or who are living with a medical condition affecting their ability to excel in daily activities and require specific complex support. We offer continuous support from a consistent team of clinicians who understand the needs of the people we serve and are trained to provide high-quality complex care. Our 24-hour support ensures that all needs are met in two shifts in 24 hours. 

Our team’s proactive approach to crisis intervention and prevention is rooted in the best training and practice that the Rapid Response Service provides.

The 24-hour support entails the following:

  • Personal Care, where we make priority people’s preferences and capacity of receiving the care.
  • Medication management – our clinicians have medication training 
  • Booking and supporting individuals to attend personalised health appointments
  • Support to promote independence by teaching life skills such as cooking, cleaning, meal preparation, etc.
  • Positive Behaviour Support Plans – proactive and reactive strategies
  • Ensuring people have well-structured daily routines, engage in outdoor activities, run errands, and support individuals in having work experiences that align with their capabilities.

We want the vulnerable people we support and their families to feel more comfortable and secure with our expert clinicians.
