Nurse-Led Care
Our nurses often spend more time with the people they support, allowing them to understand their needs on a deeper level. This is particularly important when it comes to long-term care, where supported people may require regular monitoring and support.
Did you know? 70% of people prefer to see nurses for follow-up care because of the time and attention they receive.
But what exactly do nurse-led teams do?
Rapid Response
Our Rapid Response Service offers swift, expert support for people experiencing a crisis, sudden change in needs, or risk of placement breakdown. Available at short notice, 2-4 hours since initial call, our specialist team works closely with individuals, families, and professionals to stabilise scaling situations, reduce distress, and avoid unnecessary hospital admissions. We prioritise safety, dignity, and continuity of care, providing short-term, tailored interventions focusing on the person’s unique needs. This responsive approach ensures timely solutions and helps maintain stability in the most challenging moments.
What Families Say
“The thing that sets the UCS team apart from other care providers is that they genuinely care and provide humanised care. It’s the little things that stand out – buying Elijah a gift for his birthday or for Christmas. It makes us feel valued as a family and a client.“

“William’s world has expanded a lot since the care started; it was very limited up until then. He has now, and this is, you know, it’s hard to believe that I’m saying this: William, in the last 18 months, has been on a train for the first time. William’s been on a train now, which was lovely to see. Seeing him get off the train on the platform, it was like he looked like a kid at Christmas, it was, his eyes were everywhere, and he was smiling, he was so happy.”

“My son George had issues with alcohol abuse. The clinicians from Unique Community Services helped him get sober and take control of his life. They improved his overall health, and with the behavioural treatments, he got to feel better and lead a better life. The clinicians continue to check up on George; their support means everything to him and the whole family.”

“Our daughter Charlotte has autism. Her behaviour has improved since we started working with the clinicians at UCS. Charlotte is calmer thanks to all the practices and the consistency of expert care that the clinicians offer. She is happier, and we are so grateful.”

“My brother has autism and learning disabilities. Our experience with UCS has been very positive. I’m very grateful to the clinicians and their devoted work in taking personal care of my brother’s health, speech, hygiene, safety, and independence around the house. Now he’s very keen on doing many activities outside the house”

“My husband John has dementia along with depression. For the past eight months, the clinicians from Unique Community Services have been so patient and kind. They have managed to help my husband with so many challenges that he faces daily. “

“Our son James has autism. When he was young, it was easier to control his challenging behaviours. As he got older, it got more complicated when he felt strong emotions. At Unique Community Services, they knew how to handle everything. They delivered the care and support we all needed. I feel like I can be James’s mum again and connect with him as a parent.”
